Parent/Guardian Consent, Participation Agreement & Media Consent

I, as the parent or legal guardian of the child(ren) registered above, give permission for my child(ren) to participate in Zumba® Kids classes organized and instructed by Alaa Khaireldin.

I understand that participation in dance, fitness, and physical activity involves inherent risks, including but not limited to slips, falls, collisions, muscle strains, and other minor injuries. I acknowledge these ordinary risks and voluntarily allow my child(ren) to participate.

I confirm that my child(ren) are in suitable physical condition to participate in the classes. I agree to notify Alaa Khaireldin in writing of any medical conditions, allergies, injuries, disabilities, medications, or other circumstances that may affect participation or require special consideration.

In the event of an accident or medical emergency, and if I cannot be contacted promptly, I authorize Alaa Khaireldin to seek appropriate medical assistance for my child(ren) as reasonably necessary.

I understand that the instructor will take reasonable measures to provide a safe environment and appropriate supervision during the scheduled class period. Nothing in this agreement excludes or limits liability where such exclusion or limitation is prohibited by applicable law.

Parents/guardians are responsible for ensuring that their child(ren) arrive on time and are collected promptly at the end of the scheduled class. The instructor's responsibility for supervision applies only during the scheduled class period.

I understand that bookings are non-refundable. A cancellation made at least 24 hours before the scheduled class start time will receive a full class credit. Cancellations made less than 24 hours before the class start time, or failure to attend, will result in forfeiture of the class fee.
